New Patient Consultations

Comprehensive New Patient Intake Appointment (60 minutes)
Includes full biopsychosocial assessment, diagnostic clarification, treatment planning, and coordination with referring providers (as appropriate).

Follow-Up Appointments

Follow Up (50 minutes)

Other Services

Care coordination, phone consults, and letters/forms: Prorated at $150 per 15 minutes

Extended consultations (by request): Prorated at $150 per 15 minutes

No cost for brief phone calls under 5 minutes

Insurance & Reimbursement

We are an out-of-network provider and do not bill insurance directly. Upon request, we are happy to provide a superbill for you to submit to your insurance for possible reimbursement. We recommend checking with your insurance plan about your out-of-network benefits.

Payment

Payment is due at the time of service. We accept major credit cards, HSA/FSA cards, and debit cards.
