Comprehensive Intake Appointment (60 minutes)
Includes full biopsychosocial assessment, diagnostic clarification, treatment planning, and coordination with referring providers (as appropriate).

Follow-Up Appointments

Substance Use Follow Up (50 minutes)

Eating Disorder Follow Up (50 minutes)

Brief Eating Disorder Follow Up (30 minutes)

Gynecologic Follow Up (30 minutes)

Other Services

Care coordination, phone consults, and letters/forms: Prorated at $150 per 15 minutes

Extended consultations (by request): Rates available upon inquiry

No cost for brief phone calls under 5 minutes

Insurance & Reimbursement

We are an out-of-network provider and do not bill insurance directly. Upon request, we are happy to provide a superbill for you to submit to your insurance for possible reimbursement. We recommend checking with your insurance plan about your out-of-network benefits.

Payment

Payment is due at the time of service. We accept major credit cards, HSA/FSA cards, and debit cards.
